Goods Movement Emission Reduction Funding Program (Proposition 1B) Heavy-Duty Trucks and Transport Refrigeration Units (TRUs)

Project Type	Additional Forms to Complete
Truck Replacement	Form B1 (PDF, 693KB) for each truck included in the application
Two-for-One Truck Replacement	Form B1 (PDF, 693KB) and B2 (PDF, 665KB) for each two-for-one truck replacement included in the application
Three-Way Truck Transaction	Form B3 (PDF, 699KB) for each 3-way truck transaction in the application
Truck Stop Electrification Infrastructure	Form C1 (PDF, 372KB)
Electric Charging Stations or Hydrogen Fueling Units	Form C2 (PDF, 290KB)

For Heavy-Duty Truck Projects: Provide the required supporting documents, including but not limited to:
1. CARB Truck and Bus Rule Compliance Certificate
2. Title
3. A photograph of the truck manufacturer's label specifying the Gross Vehicle Weight Rating and Vehicle Identification Number (VIN)
4. The following documents for the past 24 months:
- Mileage documentation
- CA DMV Registration
- Proof of Insurance
Complete and Submit ALL Business Request Forms:

2. Attachment 2 - Disadvantaged Business Certification (PDF, 42KB)
3. Attachment 3 - W-9 Request for Taxpayer Identification Number and Certification (PDF, 1.2MB)

5. Attachment 5 - Campaign Contribution Disclosure (PDF, 109KB)
6. Attachment 6 - Certificate Regarding Debarment, Suspension, and Other Responsibility Matters (PDF, 25KB)
